Transition support unit

We support people with mental illness and cognitive or intellectual disability to improve their living skills so that they can lead the life they want.

About us

The Transition Support Unit (TSU) is a 13-bed unit.

People from anywhere in Victoria can use this service. Most people stay for 3 to 5 years. How long you stay will depend on how quickly you get to where you want to be.

When you arrive, you will be given a key worker. This is the person who will help you identify your recovery goals and support you to achieve them.

You will have a number of other people involved in your care. These include a doctor and a psychiatrist.

You will have regular meetings with your doctor, psychiatrist, key worker and the manager of our unit while you are staying here. We will work with you on your treatment and recovery.

The unit

You will have your own lockable bedroom with a bathroom. All bedrooms have a private desk, lockable safe and TV.

Meals are provided. You can make your own meals and cook with staff assistance if you wish.

There is a phone available for you to remain in contact with people important to you.

If you feel stressed or unsafe, we have a sensory room. It's comfortable, private and has things to help you relax, such as music, calming images and sounds.

Our services

We provide the following services:

  • Treatment and support
  • Medium to long-term supported residential rehabilitation focused on your recovery goals
  • Access to a team that includes psychiatrists, psychiatric nurses, doctors, a social worker, an occupational therapist and a consumer consultant
  • Recovery and discussion groups
  • An activities program to help your recovery. This will include opportunities to do your own shopping, laundry and socialising with others. In addition, activities like outings, exercise, relaxation skills and expressing yourself through art or music feature on the program.
