Helping you stay active & well at home
Our Health Independence Program (formerly known as Hospital Admission Risk Program (HARP), Subacute Ambulatory Care Services (SACS) and Post-Acute Care, PAC) provides short term services for people with health problems and other complex conditions.
We provide care and support for people to optimise their function and self-management skills, so that they can live independently and well in their homes and community.
The Health Independence Program encompasses many of the services that deliver health care to support the transition from hospital to home or prevent the need for a hospital presentation or stay, with people able to access them directly from the community.
Most patients attend on an outpatient basis but there are some services that provide support at home.
Austin Health also has services that may provide an alternative to an inpatient stay, including Hospital in the Home and the Ambulatory Care Centre.
