Managing heart failure

We help people manage their heart failure.

Our Heart Failure Outreach team supports adults in our local community to manage their heart failure.

We can visit clients in their home. Clients can also choose to come in for appointments.

Our team of specialist heart failure nurses provide the following services:

Phone and home visit follow-up, to support clients following a hospital stay
Education and information for clients and carers to encourage self-monitoring and self-management of their heart failure
Liaison with the client’s GP and cardiologist

GP’s, specialists, family, carers, case managers and patients can contact us directly to access our services.

Open hours: Monday to Friday from 8.30am to 4.30pm (excluding public holidays)
