Community Rehabilitation Services

We are a short-term, goal-based service that supports people in the community to meet their rehabilitation goals through an individualised therapy program.

What we do

Community Rehabilitation Services (CRS) help people in the local community improve their function and independence through an individualised, short-term, goal-based therapy program.

We care for and support people with a wide range of conditions including:

  • Nerve, spine and brain (neurological)
  • Bones and joints (orthopaedic)
  • Falls and balance problems
  • Age related changes
  • Amputations

We work with you to establish rehabilitation goals and an individualised care plan is developed, with an emphasis on teaching you and your carer/s about how to manage your condition.

Your program may include individual and/or group therapy, and take place at the hospital, in your home, via telehealth or a combination.

The length of your program depends on your goals and capabilities; however we do not provide long-term therapy or maintenance therapy.

Our Community Rehabilitation Services (HIP) are part of the suite of services provided by the Health Independence Program (HIP).

What to expect

An initial assessment, usually over the phone, will be conducted to discuss your needs and goals. Together we will establish a therapy plan that will best address your needs.

What to bring

When you come to hospital for your appointment, you need to bring:

  • Medicare card
  • Adverse drug alert card (if you have one)
  • Medicines you need to take while you are here
  • Glasses, hearing aid, walking frame (if applicable)

 

Referrals

For patients

Ask your GP, specialist, or health practitioner to organise a referral for this service.

Family, carers, case managers and patients can contact us directly to access our services.

Wait time

The current wait time for this service is 2 to 6 weeks from receipt of referral.
