It is essential when completing Nursing Documentation that you are concise, factual, and objective.
When do I document?
- Daily (on AM/PM or night shift).
- At any time of a significant clinical change in condition ie. transfer or post operatively.
Our nurses follow an ISBAR or systems approach to documentation that we ask you follow when writing your notes.
