Managing medication

We help people recently discharged from hospital to manage their medications.

About us

Our Health Independence Program (HIP) provides care and support for people to optimise their function and self-management skills, so that they can live independently and well in their homes and community.

We provide many of the services that deliver health care to support the transition from hospital to home or prevent the need for a hospital presentation or stay. The services are also able to be accessed directly from the community.

Most patients attend on an outpatient basis but there are some services that provide support at home.

Our services

We provide the following services:

  • Manage complex medication regimes
  • Provide further education about medications
  • Manage known or suspected medication issues
  • Liaise with GPs and community pharmacists

Referrals

You can access this service when you are staying in hospital. Ask a member of your care team to contact us.

If a patient is well known to another health service, the Medication Management team may refer the patient onto a similar service at another hospital.
