What to expect
The Amputee Clinic is run at Royal Talbot Rehabilitation Centre on Monday afternoons.
Our service offers support to assist in the following areas:
- pre-amputation assessment and counselling
- prosthetic and non-prosthetic assessment
- ongoing prosthetic management
You will see a rehabilitation medicine doctor and a prosthetist. Our team will assess and discuss with you how we can assist with your recovery and return to everyday life following an amputation.
We work closely with other health professionals to ensure our patients achieve the best outcome during their rehabilitation journey.
What to bring
When you come to hospital for your appointment, you need to bring:
- Medicare card
- Health care card/concession card (if applicable)
- NDIS plan contacts – Support Co-ordinator, Plan Manager (if relevant)
- TAC or Workcover claim number (if relevant)
- Adverse drug alert card (if you have one)
- X-ray films, scans, ultrasounds or any other test results or reports
- Medicines you need to take while you are here
- List of medicines you are currently taking (or the boxes) including herbal supplements and vitamins
- Glasses, hearing aid, walking frame (if applicable)
Referrals
For patients
You need a referral from your GP, specialist or health care provider to access this clinic.
For health professionals
- Refer your patient to: Dr Yan Chow - Rehabilitation Medicine Physician
- We accept GP and specialist referrals

What to include in your referral
Referrals should include:
- Relevant clinical history
- Reason for referral
- Patient details including address, date of birth and contact phone numbers
- Your details and provider number
- Name of the consultant (for Medicare clinics)
- Investigation reports related to the referral
Referrals are triaged based on priority.
