Key information for referrers

Please refer to the inclusion/exclusion criteria and ensure that all requested investigations and relevant clinical history are attached to your referral to support appropriate triage.

• Incomplete referrals or missing results may lead to delays or declined triage.
• Patients must be made aware of the time-limited, shared care model.
• This is an endocrinology-only service. Mental health and surgical pathways must be managed externally.

Inclusion criteria

• Persistent gender incongruence or gender dysphoria.
• Demonstrated capacity to provide informed consent for medical treatment, including an understanding of risks, benefits, and alternatives to hormone therapy.
• Any significant medical or mental health conditions must be reasonably well-controlled prior to referral.

Exclusion criteria

• Patients under 18 years of age.
• Patients unable to provide informed consent, due to cognitive, psychological, or medical impairment.
• Patients with unstable or complex medical or psychiatric conditions, including:
  • Psychosis
  • Dissociative identity disorder
  • Complex trauma
  • Personality disorders
  • Bipolar disorder
• Patients requiring co-located support from gender-affirming mental health, psychology, psychiatry, or allied health services.
