How to make a referral

We accept referrals from a variety of sources including individuals, family members, carers, support coordinators and health professionals. You can submit referrals using the referral form below via mail, email or fax.

Referrers will need to provide supporting health information or details of where this information can be obtained. Documentation of the brain injury is required, for example:

  • neuropsychological assessment
  • rehabilitation discharge summary
  • medical imaging such as MRI.

The individual may be advised to see their GP or other local services prior to referral, however our intake worker is available to discuss the situation and make a recommendation based on your individual circumstances.

If you need urgent assistance

If you need urgent mental health assistance, we recommend that you contact your local mental health service. To find a service in your area, visit the Victorian Government's Find a mental health service.

If you need immediate emergency assistance, you should telephone 000.
