Memory Clinic

Memory Clinic

We provide a specialist diagnostic service for people who are concerned about changes to their memory and thinking. The aim is to ensure a timely diagnosis.

What we do

Our Cognition, Dementia and Memory Service (CDAMS) is a multidisciplinary team of health professionals with expert knowledge and experience including geriatricians, nurses, neurologists, speech pathologists and neuropsychologists.

We provide the following services:

  • expert clinical diagnosis
  • information on appropriate treatments
  • education, support and information
  • direction in planning for the future
  • information about dealing with day-to-day issues
  • linkages for clients and their family to others service providers or community supports.

CDAMS is part of the suite of services provided by the Health Independence Program (HIP).

What to expect

The initial appointment will include an assessment of cognitive and functional status, memory testing and a physical examination.

Clients are encouraged to come with a support person as the Collaborative History is an important part of the assessment.

What to bring

When you come to hospital for your appointment, you need to bring:

  • Medicare card
  • Adverse drug alert card (if you have one)
  • Medicines you need to take while you are here
  • List of medicines you are currently taking (or the boxes) including herbal supplements and vitamins
  • Glasses, hearing aid, walking frame (if applicable)

 

Referrals

Ask your GP, specialist, or health practitioner to organise a referral for this clinic.

Family, carers, case managers and patients can contact us directly to access our services.

Wait time

The current wait time for this service is 2 to 3 months from receipt of referral.
