Chronic Wound Service
Our Chronic Wound Service provides a consultation service to patients who have a had a wound longer than 4 weeks.
The appointment consists of an in-depth assessment to assist with establishing a wound diagnosis and treatment plan. Education and support are offered to optimise healing outcomes and support independence in the community.
It is multidisciplinary clinic consisting of clinicians with expert knowledge and experience in wound management including geriatricians, aged care registrars, nurses, podiatrist and a dietician.
It is part of the suite of services provided by the Health Independence Program (HIP).
All patients are assessed by our multidisciplinary team at their initial consult. Different diagnostic tools and tests are utilised to assist with establishing a wound diagnosis and plan of care. These tests may include dopplers, blood tests, biopsies, x-rays or other scans.
A treatment plan is established at each consultation and a copy provided for the service provider attending to the care. Three dressing changes are given to the patient until they can obtain more stock supplies.
All consultations are bulk billed via Medicare.
Review appointments are re-booked based upon clinical need and assessment.
Patients are encouraged to come with a support person if needed.

Anyone with a wound for longer than 4 weeks is eligible to attend the clinic. Appointments are based in the Outpatients department at the Heidelberg Repatriation Hospital.
You will need a referral from your GP or specialist to access the clinic.
