The Transplant Library, produced by the Centre for Evidence in Transplantation, provides access to high quality evidence-based information on all aspects of solid organ transplantation. It includes all randomized controlled trials from the earliest record, including many congress abstracts and selected systematic reviews from 2008. RCTs published from 2004 have been given a methodological quality rating.

The Transplant Library can be accessed from the Library Hub page under "Decision Support or Point-of-Care Tools".

Improving the safety and quality of care for patients with cognitive impairment (dementia and delirium) is the focus of new resources launched this week by the Australian Commission on Safety and Quality in Health Care. These resources describe a pathway to improve the early recognition and response to patients with cognitive impairment.

Commission chair Professor Villis Marshall stated "patients with dementia are two times more likely to experience falls, pressure injuries or acquire infections while they are in hospital. They are also six times more likely to develop delirium".

The resource provides evidence-based actions for clinicians and is available as an interactive e-resource or downloadable as a PDF and will be provided as an App in 2015.

The Australian Commission on Safety and Quality in Health Care and NPS MedicineWise have developed an online learning module on taking an accurate and complete medication history when patients are admitted to hospital.
Get It Right! Taking a Best Possible Medication History is designed for junior medical officers, nursing and pharmacy staff and uses a video to guide clinicians on how to obtain and record a best possible medication history (BPMH). The module explains what information should be recorded, why at least two sources of information are used to obtain the BPMH and the reasoning behind how medication history taking techniques can influence the accuracy of the history obtained.

Researchers have developed a strategy to assess the validity of clinical guideline recommendations to estimate how long it takes for recommendations to become outdated.

92% of the recommendations were valid 1 year after their development but this probability decreased in subsequent years to 77.8% after 4 years. The authors conclude that "recommendations quickly become outdated, with 1 out of 5 recommendations being out of date after 3 years. Waiting more than 3 years to review a guideline is potentially too long."

Released in October 2014, this guideline covers the care of adults who have a diagnosis of acute heart failure, have possible acute heart failure, or are being investigated for acute heart failure. It includes the use of ventilatory support and pharmacological therapy, the role of specialist management units, and the role of early natriuretic peptide testing.
