Advance Care Planning

Advance care planning is a process of planning for future health and personal care whereby a person's values, beliefs, and preferences are made known so they can guide clinical decision making at a future time when that person cannot make or communicate their decisions.

The Advance Care Planning (ACP) department at Austin Health delivers patient services, education and training across the organisation and externally. Since 2002, Austin Health has worked extensively with other health services to provide training and foster innovation in the field.

We provide:

  1. advance care planning services for consumers and patients across Austin Health;
  2. education and training for Austin health professionals;
  3. implementation of quality programs and standards;
  4. research and innovation project management.
