Chronic Disease Management

Provides a multidisciplinary service to clients with chronic respiratory disease and chronic heart failure.

The program employs a team of doctors, nurses and allied health staff, working with GPs and local community health centres.

The program facilitates patient education and encourages self-management.

Services within the Chronic Disease Management program include:

Outreach Nursing

Specialist heart failure/respiratory nurses perform home visits, providing education and support, encouraging the client with self-management of their chronic illness.

Eligibility:

Austin Health clients with a diagnosis of moderate to severe heart failure as per the Heart Failure guidelines and/or chronic respiratory disease as per COPDX.

Clients must reside in the local government areas of Banyule, Darebin and Nillumbik.

Location:

The outreach nursing staff are located at Austin Health, Banyule, Darebin and Nillumbik Community Health Services.

Heart Function Clinic

Specialist medical, nursing and pharmacy staff in the Heart Function Clinic will provide:

  • A comprehensive assessment of heart function
  • Medication review
  • Health and lifestyle advice

Eligibility:

Pre-existing or newly diagnosed systolic or diastolic heart failure.

Referral:

Clients can be referred to the clinic by their local doctor, or by a hospital doctor after a hospital stay.

Rehabilitation Programs

Includes exercises to improve strength and endurance, general nutrition advice and health information.

Eligibility:

Austin Health clients with a diagnosis of heart failure and/or chronic respiratory disease.

On-going, community based, weekly program for clients with a diagnosis of heart failure and/or chronic respiratory disease.

Eligibility:

Clients must have completed an outpatient hospital based program.

Cardiac COACH (Coaching Patients on Achieving Cardiovascular Health)

The COACH Program is an award winning telephone based training program for clients with coronary heart disease.

At Austin Health, a dietitian or nurse coach:

  • Trains clients to aggressively pursue coronary risk factor targets.
  • Works in partnership with the clients' doctors.
  • Conducts regular telephone coach calls and sends mail outs after the client is discharged from hospital.

The program is particularly beneficial to those who return to work soon after being discharged and are unable to attend cardiac rehabilitation groups.

The coaching sessions continue for a period of six months or until the target for the risk factors is achieved.

Eligibility:

This program is available to Austin Health clients who have experienced a coronary event such as:

  • Heart Attack
  • Angina
  • Stenting
  • Coronary bypass surgery

Clients need a good ability to understand and speak English.

Location:

The cardiac coach is located in the Cardiology Department at the Austin Hospital. The program is delivered by telephone.
