Discharge communication

Change to the way GPs receive discharge summaries from Austin Health

In June 2011, Austin Health introduced a new statewide IT system for health, HealthSMART. This means that GPs now receive discharge summaries in a different format.

The delivery method for discharge summaries may also have changed. Full details of the changes and the requirements of GPs are outlined in the fact sheet attached below.

What do GPs need to do?

  • To make sure that GPs can receive discharge summaries from the new sytem, they must register on the Human Services Directory (HSD) and nominate how they would like to receive discharge summaries (by post, fax or electronically).

  • To receive discharge summaries electronically, GPs must also make sure that they have a HealthLink account set up.
