Ongoing ethical approval is contingent on receipt of Progress Reports Annually. Failure to submit an Annual Progress Report may lead to suspension of ethical approval.
A Progress/Annual Report for each project must be submitted to the lead HREC and Austin Health Office for Research every year. Annual Reports are due by the approval anniversary of each study.
The following Site Report Form must be used for the Progress/Annual Report and submitted to Austin Health Governance for acknowledgment.
Once a project has been completed, a final report must be submitted to the lead HREC and Austin Health Office for Research summarising the project findings/outcomes and any unexpected developments. If available, inclusion of a published manuscript with this report is encouraged.
The final report must be submitted using the Final Report Form.
1. Please collate the Progress or Final report form and any necessary associated documentation.
2. Please submit one electronic copy with electronic signatures to ProgressReports@austin.org.au
