Updated Insurance Certificates

It is a requirement that all updated insurance certificates throughout the life of a project be submitted to the Office for Research.

Submitting your Insurance Certificate via ERM

All Insurance Certifactes must now be submitted via the Ethics Research Management website.

        1. Please log into into the Ethics Research Management website

        2. Please complete the amendment application form and all of the necessary associated documentation

       3.   Please submit your Insurance certificate to ethics@austin.org.au

[Please title the subject of the email "Insurance Certificate for [Austin Health Project Number & SERP/Protocol number (if applicable)]”
