Ongoing ethical approval is contingent on receipt of Progress Reports Annually. Failure to submit an Annual Progress Report may lead to suspension of ethical approval.
A Progress/Annual Report for each project must be submitted to the Office for Research via ERM every year. Annual Reports are due by the approval anniversary of each study.
When Austin Health is acting as the Lead Site the Project Report Form is required to be submitted on behalf of the project by either the PI or Sponsor. A Project Report Form must be completed by Each Participating Site and submitted with the Project Report Form to ProgressReports@austin.org.au.
If the project is a Single Site Project being undertaken at Austin Health it is only necessary to complete and submit the Project Report Form

Once a project has been completed, a final report must be submitted to the HREC summarising the project findings/outcomes and any unexpected developments. If available, inclusion of a published manuscript with this report is encouraged.
The final report must be submitted using the Project Final Report Form and submitted via ERM.
All progress and final reports must now be submitted via the Ethics Research Management website.

2. Please collate the Project Progress or Site Closure of Project Final report form and any necessary associated documentation.
3. Please submit one electronic copy with electronic signatures to ProgressReports@austin.org.au
